If a president is convicted on state charges and sentenced to state prison, I severely doubt the federal government would hand him over. It's likely that the Supreme Court would rule against the state if it went to court.
They would see it as a problematic precedent to allow a state to jail a president. They'd probably rule that presidents are completely immune from state prosecutions and that all criminal matters must be handled federally. Even there, I have to suppose they would probably say that a sitting president cannot be imprisoned and that it is up to congress to remove them from office. They would pribably also not prevent a self pardon, as the constitution places no limits on it.
If he wins, he probably goes free. If he loses, he probably gets home confinement at worst. Logistically, sending a former president to prison is a nightmare, so it is unlikely that happens.
